Here's a control center for a Rock-Ola 480.  Compare part number.  
Might be just what you need, or a good back-up.
ALWAYS good to have spare components for your juke around, especially when they're cheap. 
Except for my G-120, for example, I have a spare amp for all my jukes, and various other components.  
Most I paid $10-$20 for and by a stroke of luck, all good.
